| On this screen, give your connection an appropriate name (i.e. CableONE) and verify your dial-up modem is selected in the drop down box. Click Next. (View Screenshot) |

| On this screen, type in the area code 602, Telephone Number 340-1800 and verify the Country Code displayes the United States of America. Click Next. (View Screenshot) |
